how to add attachments to report program

       hi experts...

           I am doing attachments to cusrtom report for this requirment i need to create attachment icon.In this icon two items there 1.create attachments

2.attachment lists

when i click on create attachment then it will create one attachment

when i click on attachment list it will show u all created attachments

plz guide me how to get this requirement   with sample code..
